Thoracic extension and shoulder ROM
Notes
Thoracic extension and shoulder ROM
One-line summary: Multi-week thoracic-extension or multimodal programs were tied to FHP or glenohumeral ROM in retrieved abstracts; one-session thoracic manipulation reduced IR gains from local shoulder stretching.
This page is research synthesis, not medical advice. Thoracic work is in scope only because the papers tie it to a shoulder outcome.
The insight
Two different thoracic interventions appear in this pass and they do not agree. Weeks of thoracic-extension exercise (office FHP) or a multimodal spine/shoulder program (tennis) moved posture or GH ROM. A single HVLA thoracic manipulation immediately before sleeper stretch or glenohumeral mobilization reduced IR-PROM gains. From 2026-08-22-academic-research-effective-shoulder-exercises-strength.
Evidence
- From 2026-08-22-academic-research-effective-shoulder-exercises-strength: Kang et al. 2021 — scapular-stabilization plus thoracic-extension exercise in office workers with FHP improved craniovertebral angle, pain, and disability; the abstract ties thoracic extension to a shoulder/posture office-worker question.
- From 2026-08-22-academic-research-effective-shoulder-exercises-strength: Le Solliec et al. 2023 (n=22 competitive young tennis players, 8 weeks, 4×/week multimodal stretch/strengthen/myofascial program) — increased thoracic mobility, decreased interscapular distance, increased GH internal and external rotation ROM; the preceding 8 weeks of regular tennis training alone did not move those measures.
- From 2026-08-22-academic-research-effective-shoulder-exercises-strength: Swanson et al. 2024 — adding a single-session thoracic manipulation at T3–4 reduced the IR-PROM gain from sleeper stretch or posterior glenohumeral mobilization (mean differences 6.4° and 4.4°).
- From 2026-08-22-academic-research-effective-shoulder-exercises-strength: “Thoracic extension exercise over weeks (Kang 2021; Le Solliec 2023) is not the same intervention as a one-session HVLA manipulation. Do not collapse them.”
Contradictions / tensions
- Office-worker thoracic extension vs tennis multimodal vs one-session manipulation: different populations, doses, and techniques. One tennis study is not an office-worker result.
