Why it matters

Opened from the research queue (Paul via Leo). Research only: which exercise kinds the peer-reviewed abstracts support for strength, mobility, and desk/posture. Not a diagnosis, not a product, not a rehab protocol.

What we currently believe

Still open. Retrieved Consensus abstracts support three kinds of work — progressive resistance training, named mobility drills, and stretch-plus-scapular desk/posture programs — but they do not name a single best gym list and they do not license a treatment plan.

Strength: whole-body RT overviews report heavier loads, 2–3 sets, and ≥2 sessions/week beat no exercise (progressive-resistance-training). Shoulder-specific naming is thinner: sidelying ER and prone horizontal abduction at 100° with full ER rank highest on EMG (shoulder-external-rotation-exercise); one small isokinetic study reports eccentric ER gains. That is not an overhead-press / lateral-raise ranking.

Mobility: sleeper stretch and cross-body stretch (alone or with dorsal-glide mobilization) increased IR and horizontal-adduction PROM (glenohumeral-mobility-stretching). A 7-week pec-major stretch increased extension ROM in healthy adults (pectoralis-stretching).

Desk / posture: a 2024 meta-analysis reports therapeutic exercise reduced forward-head, rounded-shoulder, and kyphosis angles in people labeled with upper-crossed syndrome — a paper label, not a diagnosis here (rounded-shoulder-posture; desk-sitting-to-rounded-shoulder-corrective-exercise). Office-worker RCTs name scapular stabilization, thoracic extension, pec-minor stretch, and lower-trap work. Thoracic extension over weeks and one-session thoracic manipulation do not agree (thoracic-extension-and-shoulder-rom).

Evidence we need

  • A shoulder-specific bake-off of named gym lifts (press, lateral raise, row) in healthy adults — not returned in these abstracts.
  • Full-text exercise lists from Sepehri 2024 and the office-worker RCTs (abstracts omit protocols).
  • Whether multi-week thoracic extension and one-session thoracic manipulation can be compared on the same IR-PROM outcome.

How to resolve

Do not treat this pass as a workout prescription or a diagnosis. Leave the question open until a later pass either (a) finds a named-lift ranking in full text or another academic run, or (b) Paul decides the kind-level answer (RT + sleeper/cross-body/pec stretch + scapular/pec pairing) is enough. Do not invent DOIs. Do not promote encyclopedia pages as sources. Clinical decisions stay open.
